Health & Wellness Centers
The Health and Wellness Centers are right in our participants’ neighborhoods, so they’ll feel at home every time they visit. At the center, participants may enjoy social activities like bingo, live music, and meeting with old and new friends. Nutritious, hot meals are also provided every day. Door-to-door transportation is also provided to and from the center. Plus, our team of on-site specialists provide extensive care.

Community LIFE also has a network of medical specialists covering the full spectrum of medical and health needs. Community LIFE is your primary provider and authorizes access to services through our network of providers.

Now you have an overview of the advantages of choosing Community Life. To help you dig a little deeper, here are some of the questions we often get…along with the answers.
Will I see my Community LIFE doctor at the Health and Wellness Center?

Yes! You will be able to see your doctor at the Community LIFE center. You will also be able to see other specialists, such as Physical and Occupational Therapists, Registered Dietitians, Behavioral Health, Audiologists and Podiatrists. Our centers are also equipped to draw blood and coordinate on-site x-rays.

What if I need to see a specialist that isn’t at the center?

Community LIFE has a network of medical specialists who cover the full spectrum of medical needs. These specialists include Cardiologists, Orthopedic specialists, Nephrologists and others.

How will I get to the Community LIFE center and to my outside appointments?

Community LIFE provides transportation to the center and to all coordinated doctors appointments. Our drivers will pick you up at your home, make sure you have everything you need for the day and will take you the center and outside appointments – no more waiting in the lobby or on the porch – you can wait in the comfort and safety of your home.
