DUTIES AND RESPONSIBILITIES: Under general direction and in accordance with all applicable federal, state and local laws/regulations and Community LIFE policies, procedures and guidelines this position may include duties such as the following:
-Completes initial and routinely required nursing assessments of individual participants for medication related care.
-As needed participates in the development and revision of the participants Plan of Care and 24 hour service delivery as a member of the Inter-Disciplinary Team.
-Provides oversight of medication related care of participants, to include daily medication administration review of Medication Administration Records, review of physicians order forms and medication administration sets for home use.
-Functions in the role of triage nurse (for example calling participants who do not come to the center because they are ill or who miss scheduled clinic appointments and documents findings, assessing participants for treatment or other interventions, etc.)
-Works closely with the Physician in the daily management of participant care to include maintaining a continuous flow of participants through the exam rooms, checking on sick participants and assisting with procedures as needed.
-Reviews and coordinates all consults and follow-up visits to include procedures and any needed medications.
-Coordinates care planning with other nurses and medical staff to minimize or eliminate unnecessary inpatient care and accomplish smooth transitions to and from home.

-Provides nursing care and prescribed treatments, and demonstrates nursing care to participants and caregivers/families.
Graduate of an accredited nursing school.
At least one year of experience as a Registered Nurse working with a frail elderly population, preferably in long term care, home care or community health. Experience teaching patients and other health care workers preferred.
Comprehensive knowledge of current nursing practice and theory. Knowledge of the medical, social, and emotional needs of a frail, elderly population. Effective written and oral communication skills. Strong organizational and planning skills; ability to manage multiple priorities. Basic working knowledge of Windows operating systems, e-mail, word processing.
