Home Care Coordinator

Location: Homestead

Department: Nursing (LPN/RN/CRNP)

Schedule: Mon - Fri 7:30a-4p

Summary of Job Duties:

The Community LIFE Home Care Coordinator (LPN) is responsible for evaluating the participant’s plan of care as it relates to their home care needs and making changes as indicated under the direction of the RN Manager. The person in this role conducts environmental home assessments and assists with formulation the Plan of Care for home care needs to include: Home Health Aide services, durable medical equipment, incontinence supplies and respite care.  The Home Care Coordinator works closely with the scheduling staff to coordinate aide visits. They are responsible for assisting with day to day operations of the Home Care department to include the supervision of the Home Care staff and performing/coordinating supervisory visits on home care staff. The Home Care Coordinator participates in Interdiciplinary team meetings as applicable.


Required Education:

Graduate of accredited college as appropriate for LPN.

Required Licensure:

Valid PA Driver's License. Valid PA LPN license.

Required Experience:

At least one year of experience working with a frail elderly population, preferably in long term care, home care or community health.  Experience teaching patients and other health care workers preferred.

Required Skills:


  • Knowledge of the medical, social, and emotional needs of a frail, elderly population.

  • Effective written and oral communication skills.

  • Strong organizational and planning skills; ability to manage multiple priorities.

  • Must be able to work independently and to utilize critical decision making skills.

  • Working knowledge of utilization review, quality assurance and managed health care concepts.

  • Ability to work with the interdisciplinary team approach to care for the elderly.

  • Working knowledge of local health care and geriatric service networks.

  • Basic working knowledge of Windows operating systems, e-mail, word processing.


  • Able to deliver services in a compassionate, responsive, and courteous manner. Dependable, resourceful and flexible.

  • Able to work effectively with staff, participants, providers and referral sources.

  • Interest in geriatrics and community-based programming.

  • Ability to appreciate and enjoy working with elderly individuals.
